Resumo: A termografia é um instrumento não invasivo que detecta a radiação infravermelha emitida por um corpo. É um método que não apresenta riscos para o paciente ou para o aplicador e tem como uma de suas aplicações o estudo do Fenômeno de Raynaud (FR). Esta é uma condição que mostra resposta exagerada ao frio. No caso do FR secundário, pode se chegar à ulcerações nas extremidades dos dedos e, em alguns casos, até a amputações. A termografia poderia favorecer o diagnóstico e a adoção de tratamentos precoces, desde que sejam estabelecidos os padrões de análise tais como a distinção do tempo de reaquecimento pós-estresse ao frio e faixa de temperatura do reaquecimento entre pessoas sem e com o fenômeno, parâmetros de imagem compatível com FR primário e secundário entre outras lacunas ainda existentes. Assim, o objetivo deste estudo foi analisar os padrões térmicos após estresse ao frio com a utilização da termografia para identificação da suspeita do FR. Foi realizado um estudo exploratório descritivo, com 284 voluntários (257 indivíduos não diagnosticados, 14 com diagnóstico de FR primário e 12 com diagnóstico de FR secundário). O ambiente de coleta de dados foi mantido com temperatura de 23°C, sendo utilizada a câmera térmica (Fluke Ti400) para acompanhar o padrão de reaquecimento após um protocolo de estresse ao frio (imersão das mãos até o nível do carpo em recipiente com água a 10°C durante 60 segundos). As imagens foram registradas: antes, imediatamente após a imersão e a cada 5 minutos, até que se completasse 20 minutos pós-imersão. Foram delimitadas regiões de interesse (ROI) em extremidade distal de membros superiores e metacarpos. Para a análise de dados foram traçados os histogramas normalizados para definição da função de densidade de probabilidade da temperatura absoluta e da DDD (Diferença Distal Dorsal). O tempo de recuperação após estresse ao frio foi de 15 minutos e a temperatura foi superior a 25°C na população que não apresenta reaquecimento tardio. As imagens térmicas de pacientes com FR primário apresentaram simetria em todos os dedos de ambas as mãos. Nos pacientes com FR secundário, as imagens apresentaram assimetria em 75% dos casos. Obteve-se um score a partir de três sintomas, porém a separabilidade dos grupos de indivíduos não diagnosticados com o FR foi mais consistente a partir da linha de corte da recuperação após estresse ao frio. Ao se analisar os valores térmicos dos ROI’s dedo a dedo, o polegar revela-se poupado no FR, o 4° dedo (anelar) se revelou como dedo de maior significância e o 2º dedo (indicador) como o dedo mais frio. Entende-se que a termografia pode ser usada como exame complementar, sendo que o parâmetro de temperatura absoluta apresenta melhor discriminação nos dados. A DDD apresentou comportamento inconsistente na análise térmica.
Abstract: The thermography is a non-invasive instrument that detects the body’s infrared radiation. It is a method that presents no risks to the patient or the applicator and has as one of its applications the study of the Raynaud’s Phenomenon (FR). This is a condition that shows an exaggerated response to cold temperatures. In the case of secondary RF, ulcerations can occur at the extremities of the fingers and, in some cases, even amputations. Thermography can favor the diagnosis and adoption of early treatments, provided that the analysis standards are established, such as the distinction between the poststress reheat time in the cold and the reheating temperature range between people without and with the phenomenon, image parameters compatible with primary and secondary FR; among other gaps still existing. Thus, the objective of this study was to analyze the thermal patterns after cold stress with the use of thermography to identify the suspicion of RF. A descriptive exploratory study was performed, with 284 volunteers (257 undiagnosed individuals, 14 with a primary RF diagnosis and 12 with secondary RF diagnosis). The data collection environment was maintained at a temperature of 23 ° C, using the thermal camera (Fluke Ti400) to follow the reheating pattern after a cold stress protocol (immersion of the hands to carpal level in a container with water at 10 ° C for 60 seconds). The images were recorded: before, immediately after immersion and every 5 minutes, until 20 minutes after immersion. Regions of interest (ROI) were delimited at the distal end of the upper limbs and metacarpals. For the data analysis the normalized histograms were defined for the definition of the probability density function of absolute temperature and DDD (Distal Dorsal Difference). The recovery time after cold stress was 15 minutes and the temperature was above 25 ° C in the population that did not present late rewarming. The thermal images of patients with primary RF presented symmetry in all the fingers of both hands. In patients with secondary RF, the images presented asymmetry in 75% of the cases. A score was obtained from three symptoms, but the separability of the groups of individuals not diagnosed with RF was more consistent from the cold stress recovery cut-off line. When analyzing the thermal values of ROI's finger to finger, the thumb is spared in the FR, the 4th finger (annular) was revealed as a finger of greater significance and the second finger (index finger) was the coldest finger. It is understood that thermography can be used as a complementary exam, and the absolute temperature parameter presents better separability in the data. The DDD presented inconsistent in the thermal analysis.
