Resumo: Este trabalho abordou a temática acerca do uso de estimulação galvânica vestibulococlear (EGVC) em pacientes do sexo feminino, com idade entre 60 e 75 anos, acometidas por zumbido (tinnitus) associado à perda auditiva neurossensorial (PANS), uni ou bilateral, de grau leve a moderado e submetidas ao processo de ressonância magnética funcional (RMf). Objetivou-se com tal estudo: a identificação das áreas cerebrais ativadas durante o processo de estimulação a partir da tecnologia de RMf e a respectiva análise estatística dos dados. A metodologia desta pesquisa foi a análise quali-quantitativa dos dados, através do método experimental e exploratório-descritivo do ensaio e do referencial teórico do estudo da arte. As pacientes foram devidamente selecionadas em instituição de saúde pública de Curitiba, processo este submetido à aceitação do Comitê de Ética em Pesquisa. Os exames de RMf foram realizados em parceria com empresa coparticipante do estudo. Utilizou-se para análise o software SPM (Statistical Parametric Mapping) em MATLAB. Os resultados obtidos demonstram que a eletroestimulação ativou o córtex auditivo em 40% das pacientes e que há a possibilidade de o zumbido estar intimamente relacionado ao sistema límbico e não apenas auditivo. Além disso, os resultados referem-se: à contralateralidade do hemisfério esquerdo (mais ativado) e das queixas de zumbido e PANS na orelha direita; à não ativação do pré-cúneo na paciente com maior tempo de zumbido (rede neural padrão); à não ativação do cerebelo (vestíbulocerebelo) em todas as pacientes. Não se pôde relacionar de forma concisa a idade, o tempo com zumbido e o tipo de PANS com as áreas ativadas. Este estudo elucidou a hipótese de que áreas cerebrais são ativadas após eletroestimulação com corrente galvânica nos pacientes selecionados e a análise de ativação cerebral pode servir como direcionamento para os profissionais da saúde compreenderem se há relação entre a melhora do zumbido e o uso da EGVC e por fim, o conhecimento das áreas ativadas e a correlação entre elas pode servir como motivação para demais estudos.
Abstract: This study addressed the issue of the use of vestibulocochlear galvanic stimulation (VCGS) in female patients, aged between 60 and 75 years old, with tinnitus associated with sensorineural hearing loss, uni or bilateral, of degree mild to moderate and submitted to functional magnetic resonance imaging (fMRI). The objective of this study was to identify the brain areas activated during the stimulation process, the brain mapping from fMRI technology and respective statistical data analysis. The methodology of this research was the qualitative and quantitative data analysis, through the experimental and exploratory-descriptive method of the test and the theoretical reference of the art study. The patients were duly selected in a public health institution in Curitiba, which was submitted to the Research Ethics Committee. The RMF exams were performed in partnership with the study's co-participant. Statistical Parametric Mapping (SPM) software was used for analysis in MATLAB. The results showed that electro stimulation activated the auditory cortex in 40% of the patients and that there is a possibility that tinnitus is closely related to the limbic system and not just auditory. In addition, the results refer to: contralateral left hemisphere (more activated) and tinnitus and PANS complaints in the right ear; the non-activation of the pre-cuneus in the patient with a longer tinnitus time (default neural network); to the non-activation of the cerebellum (vestibulocerebellum) in all patients. It was not possible to make a concise relate the age, time with tinnitus and the type of PANS with the activated areas. This study elucidated the hypothesis that brain areas are activated after electrostimulation with galvanic current in the selected patients and the analysis of brain activation can serve as a guide for health professionals to understand if there is a relationship between tinnitus improvement and the use of VCGS and knowledge of the activated areas and the correlation between them may serve as motivation for other studies.
